Treatment is stage-led: endoscopic removal for the earliest lesions; surgery β increasingly laparoscopic β for most; chemotherapy and radiotherapy around surgery for rectal cancer; and for the 15% with MSI-high tumors, immunotherapy can outperform chemotherapy entirely. Molecular testing (MSI, RAS/BRAF) is therefore non-negotiable before systemic treatment.
